If you have a PCP (Primary Care Physician) talk to him/her and also to your insurance company. Most insurances, especially if you are on public healthcare like Medicare or Medicaid will pay for medical transportation. If you are travelling out of state, most of the times they will require you to have a doctor's order stating that there is a need for you to travel that far. If you have a doctor this should not be a problem.
If you do not have a PCP. Often times, yes, this is a problem. First step is to contact every non-profit agency you can find for your given diagnosis. Even if they can not help you, often times they will have resources for you to contact and things for you to try or apply for.
Check to see if there are non-profit agencies for your profession. Especially if you are in the entertainment/arts industry. Many of these self-employment positions (which is essentially what you are) will have programs available to help out if you have fallen on hard times. All they usually require is proof that you made your living in this industry. They won't usually help with medical transportation, but they will assist with other bills to free up the funds for your medical travel.
